The results of these requests will only show if there is a Substantiated report on the DCS Central Registry. No case details are provided. Prospective Caregivers for Foster Care / Adoption (Adam Walsh) Checks:
The forms, DCS-1083A and CSO-1058A, are agency only forms and the results will be returned to the requesting agency listed on the form(s). Please make sure all agency information listed is provided in case there are questions or issues.
Individuals:
Department of Child Safety (DCS) information is confidential and shall not be released, except as specified by law (Arizona Revised Statutes 8-807 and 41-1959). The Department of Child Safety Records Request CSO-1036A and CSO-1036AS forms are to be used by persons who believe that a report has been made about them, a parent, guardian, custodian, a child, a person who is the subject of DCS information, an adoptive parent, a prospective adoptive parent, foster parent, other relative, or an attorney representing any of these persons pursuant to Arizona Revised Statute 8-807. Parents may request a report on behalf of their children and legal guardians may request reports on behalf of the child.
The more information you provide with your request, the faster the Department will be able to respond to your request. Please provide as much information as possible, including full names and dates of birth for the child, the parents and the alleged perpetrator (if a caregiver other than a parent).
